Document Checklist

  • The original printout of your Online Physical Presence Calculation or form CIT 0407 (PDF, 1.85 MB).
  • Colour photocopy of all pages (valid or expired) of your passport(s) or travel document(s) during your five-year eligibility period, including the biographical page.
  • Photocopies of two (2) pieces of personal identification.
  • Proof of English or French language ability (if you are 18 to 54 years old).
  • Completed Waiver Request Form [CIT 0116] and supporting documentation (if applicable).
  • Two (2) identical citizenship photos.
  • Fee Receipt for $630.00 (paid online).
  • The Document Checklist [CIT 0007] (PDF, 0.9 MB).
